ABL Health Care is seeking a full time/on-site Clinical Care Coordinator LVN Home Health to join our team, where you will be ensuring that patient care is effectively coordinated and managed. You will utilize your expertise to enhance the quality of home health care in our community.  Our office is conveniently located in Burlingame.

**Responsibilities:**

- Prepare and present monthly in-services and educational meetings for the clinical team, ensuring compliance with Medicare regulations.

- Ensure timely responses to all clinical matters through various communication channels.

- Participate in quarterly QAPI meetings and initiatives, including clinical record reviews and trend analysis.

- Identify areas for improvement and implement Performance Improvement Projects (PIPs).

- Engage actively in daily operations and regularly interact with front-line clinicians.

- Provide clinical education, including mandatory training both in the field and off-site.

- Facilitate case conferences.

- Address patient complaints and follow up with clinicians as needed.

- Review referrals prior to acceptance if required.

- Oversee clinical documentation and submission processes.

- Implement and manage communication processes with clinicians through various methods (e.g., K-Mail, secure texting, phone/office calls).

- Develop guidelines for standard care plans for common diagnoses and assess patients' risk levels for hospitalization.

- Create and enforce scheduling guidelines for clinicians and ensure effective time management.

- Review weekly reports on delayed charting, wound care, and care for high-risk patients.

- Develop an organized tracking system for wound care and high-risk patients to ensure proper care and treatment.

- Monitor and track infection control, adverse events, and incident reports, incorporating findings into QAPI initiatives.

- Engage with others in QAPI projects and encourage feedback.

- Track and analyze clinical data to meet quality initiative goals.

- Perform additional duties related to clinical, operational, and educational matters as assigned by the Director.

**Education:**

- Promote and control optimal patient care through outcome-based and best-practice care planning.

- Provide OASIS education to clinicians, emphasizing point-of-care charting, accurate documentation, and OASIS competency.

- Educate clinicians on creating individualized Plans of Care, including reviewing the frequency and duration of care.

- Offer training on software/hardware, time management, physical assessment techniques, and accurate/timely data capture, including OASIS measures and care planning.

- Collaborate with physician groups to develop protocols for commonly treated diseases.

- Stay informed about the agency’s adverse events (particularly emergency care and hospitalizations) and work with the Clinical Operations team.
